Welcome to Your Patient-Centered Medical Home

MidValley Family Practice was recognized as a Patient Centered Medical Home (PCMH) in February 2013. This prestigious honor has only been awarded to the practices in the United States who have demonstrated the highest level of quality care. Attaining this recognition required over 2 years of preparing the PCMH application, collecting data, changing office protocols, and technology upgrades. We are proud to be one of only 2 (Mountain Family Health Center) recognized practices in the Roaring Fork Valley.

Patient-Centered is a way of saying that you, the patient, are at the center of your healthcare team. Your healthcare team includes you, your healthcare providers, and their support staff. 

A Medical Home is here, at your primary care provider’s office. I your Medical Home you are part of a team that will provide comprehensive, coordinated primary care that promotes partnerships between you, your healthcare team, and your family.

Being a part of a PCMH, your healthcare provider will:

  • Work with you to improve your health – we will be proactive rather than reactive
  • Review your medications at every visit 
  • Develop a personal action plan with you to address your chronic conditions
  • Set goals with you and help you monitor your progress 
  • Use secure technology such as iPads, Videos, and a Patient Portal to engage you in your personal medical record
  • Inform you of all test results
  • Help you take control of your health by providing you educational materials, hosting group visits, and linking you to other community programs and resources
  • Provide you 24-hour access to a clinical decision-maker by phone
  • Have arrangements with after-hours care to be informed of your visit or emergent treatment within 24 hours or next business day
  • Reserve space in our schedule to accommodate same-day appointments
  • Provide an environment that allows you to share your concerns and feel respected

By choosing to participate in a PCMH, I agree to:
  • Make sure my healthcare team knows my entire medical history and other doctors I am seeing
  • Tell my doctor all of the medications I am taking including supplements
  • Know what it covers my insurance and what it does not
  • Actively participate with my healthcare team in planning my care
  • Keep my appointments as scheduled
  • Adhere to the action plan designed by my healthcare team
  • Consult my healthcare team before making my own appointment with a specialist
  • Request that any other doctor I see, send my healthcare team a report including copies of lab work, test results, and any x-rays
  • Use the Emergency Room only for true emergencies
  • Be respectful and provide feedback to my healthcare team on ways they can improve
